Heart Check

If you are over 45years (Maori/Pacific/Asian) or 55years (other) or early family history and have not had a previous free heart check, please call and make a “CVRA APPOINTMENT” (Cardiovascular Risk Assessment).
Cardiovascular disease (heart, stroke and blood vessel disease) is still the leading cause of death in New Zealand.

Smoking

Smoking affects nearly every organ and system in your body. It is the main cause of lung cancer and respiratory (lung) disease and is a major risk for heart disease, mouth, throat and other cancers.
If you would like information on how to quit, please call one of our nursing team – we are here to help.

Diabetes Annual Review (DAR)

It is important for all people with diabetes to get an annual check-up. If you haven’t had a diabetes check for a while, please call one of our nursing team for a blood test and an appointment.

Prostate Check

Prostate Cancer is the most common cancer in New Zealand men. Prostate cancer affects families. The risk of prostate cancer is higher if your close relatives have had it. If you are 40 years or older and your father or brother has had prostate cancer, talk with one of our doctors about your prostate cancer risk.

Bowel Screening

The National Bowel Screening Programme is free for men and women aged 60 to 74 years. It aims to save lives by finding bowel cancer at an early stage when it can often be successfully treated. If you are eligible to take part, you will be sent:

  • An invitation letter
  • A consent form
  • A free bowel screening test kit, with instructions on how to use it

It is important to indicate on the consent form that you want a copy of your result/letter to go back to your GP/medical practice so that your records can be update.

Advanced Care Plan (ACP)

Advance care planning is the process of thinking about, talking about and planning for future health care and end-of life care. It is about identifying what matters to you.
